New Patient Appointment

As part of the registration process you will need a 20 minutes appointment with the Healthcare Assistant or General Practice Assistant for a simple check up and to fill in a questionnaire giving details of any past medical history. This helps us to provide you with an efficient service and allows us to collect necessary details about you and your health pending receipt of your full medical records (which can take several weeks).
