Care Navigation

With effect from 13th November, our receptionists, who are our care navigators, will not automatically make you an appointment.

Instead, you will be asked a series of questions to ensure you are seen by the most appropriate healthcare professional.

Healthcare professionals may include: GPs, Nurse Practitioners, Nurses, Community Pharmacists, Physiotherapists, Mental Health Workers, Social Prescribers, etc.

You may also be signposted to other available services. Your cooperation with the reception team whilst they care navigate would be greatly appreciated.


There is no need for you to queue.

Coming along to the practice does not guarantee an appointment and does not give patients priority.

All appointment requests will be care navigated whether patients attend the practice or telephone. Thanks for your understanding.

  • Pre-bookable appointments with a doctor (up to 2 weeks in advance)

  • Same day face to face appointments with a doctor / advanced nurse practitioner

  • Online appointments (up to 2 weeks in advance)

  • Telephone call backs, pre-bookable or same day with doctors and nurse practitioners 

  • Pre-bookable face to face and telephone appointments with a nurse practitioner up to 2 weeks in advance 

  • Routine nurse appointments are available to pre-book up to 6 weeks in advance

  • Specialist mental health nurse

  • Access to first contact physio

  • Structured medication reviews with our clinical pharmacy team

          Extended Access Service - Late evenings and weekends

          Appointments with this service can be booked by contacting your usual GP Practice or contacting 111. 

           

          Out of Hours / Urgent Care Services 

          Walk-in and minor injuries services are provided in the Integrated Urgent Care Centres at the University Hospital of Hartlepool and University Hospital of North Tees.

          The 24/7, GP-led facility will be provided as an alliance by North Tees and Hartlepool NHS Foundation Trust, Hartlepool and Stockton Health (the local GP Federation) and North East Ambulance Service NHS Foundation Trust, from the University Hospital of North Tees and the University Hospital of Hartlepool.

          It will include out of hours services, where patients can see a GP or urgent care practitioner for urgent care needs or minor injuries.

          If you are unwell and need to use the service we advise that you book an appointment first by phoning 111. If you have a minor injury, you should walk into the service.

          Patients will continue to access their own GP practice as they do now. However, when local practices are closed you should call 111 for an appointment with the urgent care service.

          For your information, this new Integrated Urgent Care Service leaflet has been produced by the organisations providing the NHS 111 service and urgent care centres.

          If you are unsure if you need to see your normal GP (doctor) or attend an urgent care centre then NHS 111 will advise you.
